Anemia is one of the public health problems in Indonesia that can be experienced by all age groups from toddlers, teenagers, and pregnant women to the elderly. Riskesdas 2018 shows the prevalence of anemia in children aged 5-14 years is 26.8% and at the age of 15-24 years is 32%. This means that about 3 out of 10 children in Indonesia suffer from anemia.

To overcome this, the government has made various efforts through balanced nutrition education, food fortification, and supplementation of Blood-Adding Tablets (TTD). Iron supplementation was started in 2015 by taking 1 tablet of iron tablets per week throughout the year for young girls aged 12-18 years who are in junior high/equivalent and high school/equivalent education levels. Although the administration of TTD to adolescent girls has been carried out, the prevalence of anemia is still quite high. Many factors influence, one of which is the lack of compliance of young women in consuming iron tablets. The results of Riskesdas 2018, show that the proportion of young women who received iron tablets in the last 12 months at school was 76.2%, but only 1.4% took iron tablets as recommended.

Based on these conditions, UNICEF initiated the #AksiBergizi activity and began carrying out these activities in 2018 in Klaten and West Lombok Districts through advocacy, school and community mobilization, multi-sector coordination, capacity building, and monitoring and evaluation.

The #AksiBergizi activity is carried out with three main interventions, namely (1) Breakfast and Drinking Iron together at school/madrasah every week; (2) Multi-sectoral nutrition education to promote healthy eating and physical activity; and (3) Communication for behavior change that is relevant and comprehensive. The implementation of the #AksiBergizi program is of course integrated with TRIAS UKS, namely health education, health services, and fostering a healthy environment.

The #AksiBergizi pilot project carried out in 30 schools in Klaten Regency and in 30 schools in West Lombok Regency reaped interesting results. After the intervention was carried out, there was an increase in student's knowledge about nutrition, an increase in the proportion of adolescents who had a positive attitude towards iron tablets and girls who consumed iron tablets every week, as well as an increase in adolescents who did 60 minutes of physical activity every day and consumed fruits and vegetables. The evaluation also showed that the proportion of adolescent girls who took a weekly TTD was 12 times more likely to increase after the intervention.

Activation of the #AksiBergizi Movement

In line with the results of the intervention, the #AksiBergizi Movement is believed to be one of the strategic efforts in increasing adherence to iron tablet consumption in adolescent girls, which is also an indicator of specific nutrition intervention services in accelerating stunting reduction. The #AksiBergizi movement will be carried out simultaneously in 12 priority provinces (Aceh, North Sumatra, West Java, East Java, Central Java, Banten, NTB, NTT, West Sulawesi, Southeast Sulawesi, South Kalimantan, and West Kalimantan). 

Starting the movement, the Ministry of Health activated #AksiBergizi activities in 4 selected schools in the Bogor Regency area, including SMPN 1 Cibinong, SMAN 1 Cibinong, SMAN 2 Cibinong, and SMKN 1 Cibinong. This activity involves relevant cross-sectors at the central and regional levels and all school residents. 

The day before the implementation of the activity, each school is accompanied by a local health center to carry out Hb checks/screening for adolescent girls in grades 7 and grade 10. Hopefully, this Hb examination can be carried out periodically to monitor the number of cases of anemia in adolescent girls. 

The activity which took place on Thursday, July 21, 2022, was attended directly by the Minister of Health of the Republic of Indonesia, accompanied by the Director General of Public Health, Secretary of the Directorate General of Public Health, Director of Health Promotion and Community Empowerment, Head of the Bureau of Communication and Public Services, and Spokesperson for the Indonesian Ministry of Health. 

The #AksiBergizi series of events consisted of the joint exercise, breakfast together, drinking TTD, and filling out the CERIA application together, and continued with an educational session through interactive games. The series of course does not stop on that day, but is expected to run regularly every week according to the agreement and willingness of each school. Therefore, the role of school leaders, teachers, parents, students, and all residents around the school is very important. 

It doesn't just stop at these 4 schools, the #AksiBergizi activity is also expected to be carried out in all SMP/SMA/equivalent in 12 priority provinces and even throughout Indonesia. All schools are expected to participate in carrying out #AksiBergizi activities regularly as an effort to improve adolescent nutrition and prevent anemia in adolescent girls. Of course, the success of this #AksiBergizi needs to be supported by the involvement and collaboration of cross-sectors.

Along with the activities carried out in schools, the #AksiBergizi activity was also disseminated through various social media with the hashtags/hashtags #CegahStuntingituPenting and #AksiBergiziBikinGlowing. This campaign is expected to motivate teenagers to consume iron tablets every week and eat foods that contain animal protein, such as eggs, chicken, meat, fish, etc.

Heart disease is the most common cause of death in Indonesia. Minister of Health Budi Gunadi Sadikin said that heart disease has even become the disease with the highest cost in Indonesia.

"In terms of the cost burden of the government of the Republic of Indonesia (heart disease) is also the highest. So every year, the most BPJS claims are cardiovascular," said Minister of Health Budi at the peak of the commemoration of World Heart Day 2022 at the Southeast Plaza, Gelora Bung Karno Main Stadium Complex, Senayan, Jakarta, Sunday (2/10).

In terms of costs, continued the Minister of Health, cardiovascular disease is the most common disease that makes people suffer. Heart or cardiovascular disease is also a disease that kills the most.

Currently, the government is working on an even distribution of cardiovascular specialists throughout Indonesia. The current condition is that out of 34 provinces, only 28 provinces can perform heart surgery.

“The tools already exist, the problem is that our specialist doctors are very short. We lack specialist doctors and thousands and even tens of thousands of our people die every year due to a shortage of doctors and a shortage of specialists," said the Minister of Health.

"So it is important for us to be together so that we can prevent from entering the hospital. The way to prevent it is that our behavior in life must be changed. Changed it by having to move a lot,” added the Minister of Health.

Minister of Health said Budi, the way to overcome the disease is through preventive promotions and routine physical activity, and maintaining a good diet.

“The focus should be on preventive promotive activities because when they enter the hospital, apart from being expensive for the patient, their quality of life also decreases. The most important preventive promotive activities are maintaining a diet and also maintaining physical activity," said the Minister of Health.

Overcoming the problem of heart disease cannot be exclusively carried out by the Ministry of Health, but it requires support from across ministries, institutions, and other related sectors.

Minister of Foreign Affairs Retno Marsudi is ready to support in overcoming heart problems in Indonesia. He will help if needed with cross-sectoral cooperation abroad.

"If there is cooperation with foreign countries, we are ready to help so that Indonesian heart patients do not need to go abroad for treatment, but seek treatment here because the doctors are sophisticated, the equipment is sophisticated, and the service is good," said Foreign Minister Retno.

The Deputy Minister of Health of the Republic of Indonesia, Dante Saksono Harbuwono together with the President Director of the Harapan Kita Heart and Blood Vessel Hospital, Iwan Dakota made a working visit to Cleveland, United States on 11-15 September 2022.

The agenda of this visit is to explore the potential for cooperation between RSJPAD Harapan Kita and Cleveland Clinic in terms of developing cardiovascular services in Indonesia.

During his visit to the Cleveland Clinic, the Deputy Minister of Health met with many representatives to discuss potential collaborations and to see firsthand the existing health services and facilities at Cleveland Clinic which include pre-operative rooms, post-operative rooms, intensive care rooms, operating rooms, and operating rooms. minimally invasive.

"In a special meeting session with many Cleveland Clinic representatives, we had the opportunity to discuss potential collaborations that we could develop to help advance cardiac services in Indonesia," said the Deputy Minister of Health.

Cleveland Clinic is the No. 1 cardiac care hospital. 1 in the world based on the results of a survey conducted by US News & World Reports. This ranking has been successfully maintained since 1995.

Cleveland Clinic has 9 specialist services. One of them, the Heart, Vascular, and Thoracic Institute (HVTI) which is the largest cardiovascular service in the United States. Each year, HVTI is capable of performing 50 heart transplants and serving 90-100 thousand echocardiography.

The Vice Minister of Health explained that the exploration of this partnership as a form of implementation of the second pillar of health transformation believes the transformation of referral services to improve access and quality of secondary and tertiary services, especially heart disease services so that they can be developed such as HVTI.

The Deputy Minister of Health said that currently there are not many hospitals that can provide integrated cardiovascular services in Indonesia. Of course, this raises problems, because patients have to be referred to a central hospital, as a result, the waiting time for services becomes very long. Not infrequently many patients choose to seek treatment abroad to get heart services.

Through this collaboration, it is hoped that it will be beneficial for equitable access to heart services in Indonesia.

"We must immediately carry out this collaboration to improve, strengthen and equalize cardiovascular services in Indonesia. We will develop it so that people no longer need to queue for long to get treatment," said the Deputy Minister of Health.

Indonesia's desire through RSJPD Harapan Kita to cooperate with the Cleveland Clinic was welcomed by Bill Peacock, Chief of Operations at Cleveland Clinic. Cleveland Clinic itself has previously collaborated with many universities in Indonesia related to training and organizing health seminars.

Furthermore, Rosanne Kho, Medical Directorate of International Business Development, Cleveland Clinic said that several potential collaborations can be developed between Indonesia and the Cleveland Clinic, including consulting agreements, CC Service Line, and CC Strategic Partnership as was done in China. This includes cooperation in the development of Health Human Resources which includes doctor training (observation, proctorship, seminars) and doctor-to-doctor consultation between RSJPD Harapan Kita and Cleveland Clinic.

As a follow-up to this meeting, further cooperation proposals and the preparation of a cooperation contract will include the affiliation between RSJPD Harapan Kita and the Cleveland Clinic. It is hoped that this collaboration will be realized soon.

This platform is the embodiment of the sixth pillar of health system transformation, namely the pillar of health technology transformation initiated by the Minister of Health Budi. This platform is also expected to support the implementation of the other five pillars of health system transformation, such as the transformation of primary services, the transformation of referral services, the transformation of the health resilience system, the transformation of the health financing system, and the transformation of health human resources which are currently underway.

"In the digital transformation of health, we will carry out 3 programs, firstly integrating data, secondly refining & simplifying applications, and thirdly building an innovation ecosystem. The SatuData Health program that we are launching today will integrate health data and standardize data exchange formats and protocols," said Health Minister Budi Gunadi Sadikin.

In developing this platform, the Ministry of Health adopted a Platform-as-a-service (PAAS) infrastructure model that connects the entire ecosystem of health industry players to create one reliable national health data.

Later this platform will become a liaison between various application platforms for various health industry players. For this reason, all applications and health service facilities such as vertical hospitals, government hospitals, private hospitals, health centers, posyandu, laboratories, clinics, and pharmacies must follow the standards set by the Ministry of Health on the SATUSEHAT platform.

The Minister of Health said that the IHS exchange of national health data would be more efficient and effective. Through this platform, people no longer need to carry physical medical record files if they have to move hospitals. All patient medical record resumes have been recorded digitally on the SATUSEHAT platform which is integrated with PeduliLindung and can be accessed via mobile phones, anywhere and anytime.

"Through this integration, we will integrate patient health data from all health facilities (Hospitals, Clinics, Labs, and Pharmacies) into PeduliLindungi. So that patients referred to hospitals do not need to bother sending medical documents containing lab results/diagnoses or repeating lab tests, because all data such as ultrasound, heart records, and CT scans, including drugs that have been given have been entered into PeduliLindung.” explained the Minister of Health.

Likewise with health workers, with the SATUSEHAT platform, health workers do not need to input data repeatedly in different applications. Just fill in one app, which automatically connects with other health apps.

In addition to being integrated with health service facilities, laboratories, and pharmacies, the Ministry of Health plans to integrate SATUSEHAT with BPJS Health.

Integration includes a tuberculosis recording system, a digital recording system for maternal and perinatal mortality data, immunization, a national referral system, maternal and child health, and an integrated data management information system for environmental health and disease control.

The data integration process into the SATUSEHAT platform will be carried out through several phases with the target of completing the data included in the medical resume standard to IHS.

The first phase is patient registration data and diagnosis. The second phase is medical procedure data, vital condition data, and diet data. The third phase is integrated drug data with the drug dictionary (KFA). The fourth phase, laboratory observation data and radiological observation data. Then, the fifth phase, allergy data and physical condition data.

“I hope that SatuData Health will continue to grow, integrating demographic data, medical data, and even genomic data, which we will launch next August. So that Indonesia will have the most complete and integrated digital health data system,” said the Minister of Health.

Furthermore, the Minister of Health hopes that the integration of health data will further strengthen Indonesia's health system which is more sophisticated, efficient, and effective.

Expert Staff of the Minister for Health Technology and Chief Digital Transformation Office, Setiaji said that currently, many healthcare facilities have tested this new platform. The alpha version trial has been carried out to around 41 hospitals including 9 vertical hospitals and 32 DKI Jakarta hospitals and currently undergoing beta testing of IHS in 31 institutions ranging from health companies to health labs.

"This platform has been tested on 41 government-owned vertical hospitals in the alpha testing phase and a beta phase trial is underway involving 31 institutions from different backgrounds," Setiaji said.

By the end of 2022, the Ministry of Health targets that there will be around 8,000 healthcare facilities in Indonesia that have been integrated with IHS and all of them are integrated by 2023.

Meanwhile, it is related to the security aspect. Setiaji said the Ministry of Health had collaborated with the National Cyber ​​Password Agency (BSSN) to ensure the security of users' data. This will also be strengthened by regulations from the Ministry of Health that will regulate the use of the SATUSEHAT platform.

The Indonesian Ministry of Health held blood-added tablets (TTD) for female students in Bogor Regency. This activity is an implementation of the primary service transformation launched by the Minister of Health, Budi Gunadi Sadikin.

As a first step, the provision of TTD was carried out in 4 schools, namely SMAN 1 Cibinong with approximately 630 students, SMPN 1 Cibinong with as many as 640 students, SMAN 2 Cibinong with around 600 students, and SMKN 1 Cibinong around 500 students.

TTD is given to junior high school students grades 7-9 and high school grades 10-12. Furthermore, the provision of TTD will be carried out for all junior high school and equivalent high school students in Bogor Regency.

This activity was attended by Minister of Health Budi Gunadi Sadikin. On that occasion, the Minister of Health, Budi, had a direct conversation with students in every school he visited.

The Minister of Health said that the number of mothers who died each year reached 7,800 people. One of the causes is a lack of iron, if there is a lack of iron then women are pregnant and during childbirth, the possibility of death is high due to lack of blood.

“So my job is to make sure these younger siblings are healthy and to make sure they have enough iron. It must take tablets to add blood. The tablets are taken during adolescence, before pregnancy, and after giving birth,” said Minister of Health Budi.

Minister of Health Budi advised all school students to be diligent in exercising, implementing a clean and healthy lifestyle, and eating a balanced nutritious diet.

"That's all I have left, to regularly take blood-added tablets to prevent maternal death, be diligent in exercising and eating nutritious food to be healthy," said Minister of Health Budi.

The Ministry of Health held a series of Health Working Group (HWG) meetings titled "Harmonizing Global Health Protocol Standards" in Yogyakarta on 28-30 March 2022.

As part of the G20 Presidency Sherpa Track series, this first meeting was held offline and online and attended by around 70 foreign delegates and 50 local delegates.

The HWG 1 agenda will specifically discuss one of three priority issues in the health sector in the G20 Presidency, namely the harmonization of global health protocol standards for travel between countries.

Meanwhile, the HWG 2 agenda will discuss building global health resilience. And the 3rd HWG Agenda will discuss the construction of study centers and manufacturing for prevention, preparation, and response to the upcoming health crisis.

Alignment of health protocols between countries is urgently needed to support interconnectivity and connectivity of health information systems from various countries to facilitate international travel.

Over the next 3 days, the HWG 1 meeting will be divided into 6 discussion sessions. Session 1 discusses Digital Documentation of COVID-19 Certificates, session 2 discusses Harmonizing Global Health Protocols, session 3 discusses Harmonizing Global Health Protocols, session 4 discusses Sharing National Experiences and Best Practices in Implementing Policy and Mutual Recognition, session 5 discusses Harmonizing Global Health Protocols and session 6 are closing, namely Follow Up and Concluding Plenary Session. Each session will involve experts and speakers from various countries.

Through these various sessions, it is hoped that they can produce agreements that can encourage the implementation of harmonization of global health protocols so that mobility between countries will be more secure and also accelerate the recovery of the world economy.

After the six sessions are completed, the HWG 1 agenda will be continued with the G20 Tuberculosis Side Event which will take place on 29-30 March 2022.

With the theme "TBC Control Financing: Overcoming COVID-19 Disruption and Building Future Pandemic Preparedness", this meeting became an important momentum to strengthen global commitments to end TB by 2030, especially commitments to increase funding for sustainable TB prevention and control.

The Ministry of Health through the Digital Transformation Office (DTO) held the 2022 Health Innovation Sprint Accelerator. In this activity, 5 health innovations were exhibited, especially in the fields of health technology (health-tch) and biotechnology (biotech).

The Health Innovation Sprint Accelerator 2022 is an incubation program for innovators in the health sector organized by the Indonesian Ministry of Health through the Digital Transformation Office (DTO) in collaboration with the United Nations Development Program (UNDP) and supported by the Government of Japan.

This program aims to assist innovators from startups, universities, and research institutions in developing their innovations through knowledge transfer and incubation programs.

105 participants registered for the program. From the innovation selection of the health-tech and biotech teams, 15 teams were selected to participate in roadshows in various cities including Medan, Bandung, Surabaya, and Makassar.

Until finally the 5 best teams were selected to showcase their innovative products at the Demo Day Health Innovation Sprint Accelerator 2022 on Thursday (24/3). The top three categorized as Best Innovators were selected from the judging results, including Nalagenetics (Clinical Decision Support), Femicam (FEMICAM Medical Camera), and Secepat Health (NIRGOMO). Meanwhile, the most popular innovations were won by BIGStar (Big Data in Support of Analytical Research) and TeleFuture (Vital Sense).

Nalagenetics (Clinical Decision Support) (Bio-tech) is an International standard (ISO) software that has several modules and was created to maintain the quality of product development for security and data confidentiality.

Femicam (FEMICAM Medical Camera) (Health-tech) is a medical camera made to see numbers and examine the female area, especially the cervix (cervix). This tool can help in the process of early detection of uterine cancer by seeing whether there are abnormalities or growth of cancer cells in the cervix.

As easy as Health – NIRGOMO (Health-tech) is a non-invasive self-monitoring blood glucose (PGDM) tool that uses near-infrared (NIR) spectroscopy technology to measure blood sugar concentrations in the form of a photoplethysmography (PPG) signal.

BIGStar – Big Data in Support of Analytical Research (Health-tech) is a web-based application with big data principles to collect patient medical record data that is processed into research variables.

TeleFuture – Vital Sense (Health-tech) is a medical device used to monitor vital signs at home supported by artificial intelligence (AI) programs. Health Innovation Sprint Accelerator 2022 as a means of networking between stakeholders and start-ups/innovators in various cities as well as mapping and accommodating the aspirations of stakeholders in the health-tech and bio-tech fields.

As the culmination of a series of activities, a Demo Day Health Innovation Sprint Accelerator 2022 will be held. At the peak event, selected health-tech and biotech innovators will communicate their innovations to the public and relevant stakeholders.

Indonesian Minister of Health Budi Gunadi Sadikin said the Ministry of Health encourages the use of innovative products and technology as opportunities and long-term investments to improve the quality and equity of health services.

"Through the digitization of the health and biotechnology fields, the Ministry of Health has three priorities to support the transformation of health technology in Indonesia, one of which is the development of a health innovation ecosystem, especially in the fields of technology and biotechnology," he said at the 2022 Sprint Accelerator Health Innovation Day Demo in Jakarta, Thursday ( 24/3).

Through roadshows in four cities, Minister of Health Budi hopes to be a good first step in creating a Penta helix ecosystem for digital health innovation in various regions. This is done through collaboration between the government, industry, universities, the community, and the media.
